What is it?
CDM stands for Chronic Disease Management. A chronic disease is a long-term health condition that needs ongoing treatment and management.
This programme aims to prevent and manage chronic disease, entitling you to have two extra consultations per year.

Who is it for?
This programme is FREE OF CHARGE for patient over 18 years, who have a medical card or GP visit card and have a diagnosis of one or more of the listed conditions below. This includes two sets of reviews in every 12-month period.

• Type 2 diabetes
• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Cardiovascular disease, including heart failure, heart attack (angina), stroke and irregular heartbeat (atrial fibrillation).

How do I avail of it?
If you are eligible our surgery, we will be in touch to book you in. However, if you are eligible and
have not been contacted, please get in touch by calling our reception.

What are the benefits of this programme?
• Regular reviews of your chronic disease, to enable optimal treatment and management from your GP and Nurse.
• Regular blood tests from in house phlebotomist’s Kathleen and Mary.
• Specialised services such as ECG and 24-hour ABPM from our nurse Brenda.
• A personalized treatment plan and a prescription review every 6 months from your GP.
• Early detection of any new conditions or symptoms.
• Local care avoiding unnecessary hospital visits.
• Additional Nutritional support if required from Nutritionist Susan.
